Notice of Privacy Practices
A summary of how protected health information is handled in this practice, and what rights you have over your record.
This notice describes how medical information about you may be used and disclosed, and how you can get access to this information. Please review it carefully. The full signed notice is provided with the intake paperwork before your first session.
Effective date: September 1, 2026.
How your information is used
- Treatment. To provide and coordinate your care, including with other providers you ask me to speak with.
- Payment. To bill you and to produce superbills or invoices you may submit to your health plan or HSA/FSA administrator.
- Practice operations. Scheduling, record keeping and clinical consultation or supervision, where identifying details are minimized.
Disclosures required or permitted by law
California and federal law require disclosure in a small number of situations, including suspected abuse or neglect of a child, elder or dependent adult, a serious and imminent threat of harm to you or another identifiable person, and valid court orders. Where a disclosure is required, I will tell you about it wherever it is safe and possible to do so.
Psychotherapy notes
Psychotherapy notes receive additional protection and are not released without your specific written authorization, except where the law requires otherwise.
Your rights
- To inspect and request a copy of your record
- To request an amendment to your record
- To request restrictions on certain uses and disclosures
- To request confidential communications through a particular channel
- To an accounting of certain disclosures
- To be notified in the event of a breach affecting your information
- To a paper copy of this notice on request
Outdoor sessions
Walking sessions take place in public space and therefore cannot offer the acoustic privacy of a closed office. This trade-off is discussed with you and documented in your consent before any outdoor session takes place, and telehealth is always available as an alternative.
Website and email
This website is a marketing site. It does not collect, store or transmit protected health information, and no scheduling or clinical system is embedded in it. Email and the website contact form are not secure channels.
